If you updated to v4.9 r46 and are now getting the "Table USER_ACCESS_RIGHTS already exists" error when you try to log-in, here's the file you need to fix the problem...
1. Download this file...
http://mnm.soholaunch.com/misc/v49_r46-USER_ACCESS_RIGHTS-error_patch.zip
2. Extract it on your PC....
3. Log-in to the affected site via FTP and upload the "version_compat_updates.inc.php" file contained in the zip to this folder...
sohoadmin/includes/version_compat_updates.inc.php
